Re-Admission Process
Students who have not been enrolled at the UTSPH for over a one year calendar period (total of three consecutive semesters) must reapply to the School. The following documents must be submitted to the Office of the Registrar at the University Center Tower building by October 1st. for the Spring semester and July 15 for the Fall semester:
- new application
- $30 application fee
- current goal statement, including an explanation for the leave of absence
- letter from advisor (may be the same or different) indicating his/her support of applicant's reinstatement and willingness to serve as student's advisor
- any transcripts reflecting classes taken since the original application was submitted
