New Student Checklist
Congratulations on being admitted to the UT School of Public Health! If you choose to enroll at the UT School of Public Health, be sure to complete all the enclosed forms and return them to the appropriate office identified below.

Complete Response Form
Ensure your place in the class by returning the response form if you have not already done so.
Complete Criminal Background Check
The University of Texas Health Science Center at Houston requires that a criminal background screening process be completed and submitted by all new students admitted to a degree and non-degree program prior to enrollment for their first term of study at their respective schools. Accepted students who do not consent to a criminal background check or who fail to provide the report as required will not be able to enroll in the School of Public Health. Check-in With Your International Advisor (International Students Only)
International Students should check-in with their international advisor at The Office of International Affairs (7000 Fannin, UCT Suite 130, Houston, Texas 77030) as soon as they arrive.
Attend New-Student Orientation
Attendance at orientation is mandatory for all students admitted to a degree program. Important enrollment and registration information will be given during that time. Newly admitted or re-admitted students will not be able to register until they have completed the orientation process. For more information, see the Orientation page.
Meet With Your Academic Advisor
The name and telephone number of your advisor was included in your acceptance letter. We encourage you to contact your advisor before the orientation / registration period to set-up an appointment to discuss course selection.
Complete Library User form
Complete the library user form which is distributed at Orientation day or iyou can pick it up at the Library. Please return it to the Library.
Complete Immunization form
All students must complete the UT Immunization Form prior to registration and fax it to UT Health Student Services at (713) 500-0605.
Complete Core Residency form if you have not already done so
All students must complete the core residency form prior to registration. The Office of the Registrar will not accept forms via fax. Forms must be:
- Dropped off in-person to an Office of the Registrar staff member
- Emailed as an attachment to the Office of the Registrar (registrar@uth.tmc.edu)
- Mailed to the Office of the Registrar at:

Verify Your Health Insurance Status
State law mandates that all SPH students must have health insurance. Thus, students are automatically charged for health insurance in addition to tuition and other fees. If a student already has health insurance through another source, complete the Student Health Insurance Confirmation form online or turn it in to Auxiliary Enterprises (7779 Knights Rd) no later than the 12th class day of the fall or spring semesters and the 4th class day of the summer semester. Register for classes
Pay tuition and fees
Pay Tuition and fees either online at UT-Link OR at the Payment office (located at 7000 Fannin St., University Center Tower (UCT) Room 2240). Check the Academic Calendar to view the tuition deadline dates. Registration will be cancelled for all students who have not paid by the 12th day of class.
Obtain UTHSC-ID badge
After paying fees, pose for a picture and obtain a UTHSC-ID badge in UCT 2240. Obtain current semester's sticker once you get your badge.
Complete & return UTSCH-ID Activation form
Return the form to RAS W-218.
Activate official school email address
Email address consists of first name, middle initial, followed by the last name.
E.g.: Julie Elizabeth Doe – julie.e.doe@uth.tmc.edu
Contact the UTHSC-H Help Desk (713-500-4848) for assistance in obtaining your username and password. If you are in Houston, you may also visit members of the information technology staff on the West side of the 8th floor in the main SPH building. You should also download the “Student Information Technology Handout” located on the IT Services website, http://www.sph.uth.tmc.edu/itservices.
