Roles and Responsibilities - Student

Completes the online orientation and quiz
In consultation with advisor selects a site and project
Completes and submits Learning Contract with all signatures. Forward one copy of the Learning Contract to: community preceptor, faculty sponsor, and Office of Public Health Practice/Regional Campus Coordinator. The Office of Public Health Practice/Regional Campus Coordinator must have a signed copy before the practicum starts.
Registers for PH9997
Functions professionally at the site
Complies with policies and procedures of the organization
Completes assignments related to the practicum
Maintains communication with faculty sponsor as needed
Participates in online seminar
Posts abstract on Blackboard
Completes an evaluation of the practicum
