Using email, these forms are distributed to the student and community preceptor near the end of the semester.

Waiver

Used to request a practicum waiver.

The Learning Contract is used to define learning objectives, methodology, timelines and final product. This form is signed by the student, faculty sponsor, and community preceptor. Each retains a copy, and the student forwards a copy to the Office of Public Health Practice/Regional Campus Coordinator.
The Student Evaluation is used to assess the effectiveness of the learning experience from the student's perspective.
The Preceptor Evaluation is used to assess the student's ability to accomplish the objectives and learning experience. The evaluation goes to the Office of Public Health Practice immediately upon submission and then forwarded to the faculty sponsor.
