Womenshealth.gov Quick Health Data Online 
The system provides county-level data for all 50 states, the District of Columbia, and US territories and possessions. Data are available by gender, race and ethnicity and, to the extent possible, age.

Information about research datasets and instruments/indices employed in Health Services Research, Behavioral and Social Sciences and Public Health with links to PubMed.

Archival Research Catalog (National Archives and Records Administration)
The Archival Research Catalog (ARC) is the online catalog of NARA's nationwide holdings in the Washington, DC area, Regional Archives and Presidential Libraries. Be sure to select Data Files under Type of Archived Materials.

HCUPnet (Agency for Healthcare Research and Quality)
"HCUPnet is a free, on-line query system based on data from the Healthcare Cost and Utilization Project (HCUP). It provides access to health statistics and information on hospital stays at the national, regional, and State level."  To look up data by disease classification, you will need to know the ICD-9 (NOT ICD-10) codes.  You can find a list of codes at CDC Wonder Compressed Mortality 1979-1998.  Go to Section 4: Select Cause of Death to view the categories.  Click on on item, then click on Open to view sub-categories or click Open Fully to view further down the diagnostic tree.
