Checklist for New Certificate Students
Please ensure that all tasks are completed before classes start.
- Complete Criminal Background Check
The University of Texas Health Science Center at Houston requires that a criminal background screening process be completed and submitted by all new students admitted to a degree program prior to enrollment for their first term of study at their respective schools. Accepted students who do not consent to a criminal background check or who fail to provide the report as required will not be able to enroll in the School of Public Health. - Complete Immunization form
Fax the completed form to UT Health Student Services at (713) 500-0605.
- Register for classes
View the Course Schedule to see available classes. Students must sign up for classes through UTLINK at https://utlink2.uth.tmc.edu/. To register, you will need your Student Identification number or A# which starts with the letter A and followed by 9 numbers AND your Personal Identification number ( PIN) which is a 4-digit number that consists of your birth year followed by your birth month. For example, if you were born in January 1980, your PIN would be 8001. You’re A# can be obtained at the Registrar’s Office by dialing 713-500-3361.
- Pay tuition and fees
Payment can either be made via UTLINK (http://utlink.uth.tmc.edu) or at the Bursar’s Office located at the University Center Tower (UCT) located at 7000 Fannin St. in room 2240. The Academic Registration Calendar will have important deadlines.
- Obtain UTHSC-ID badge
For Houston students, after paying fees, pose for a picture and obtain a UTHSC-ID badge in UCT 2240. Obtain a “Fall 2008”, or appropriate semester, sticker once you get your badge. Online students do not need to obtain I.D. badges. All others, please contact your respective Student Coordinators in regard to obtaining an I.D.badge.

- Complete UTSCH-ID Activation form ( Houston students only)
Download the SPH ID Card Activation form. Complete and return the form to the School of Public Health, RAS W-218.

Email address consists of first name.middle initial.last name. EX: Julie Elizabeth Doe – julie.e.doe@uth.tmc.edu. Students enrolled in online courses should check their UTSPH email account frequently for course information.
- Verify insurance status
State law mandates that all SPH students must have health insurance. Thus, students are automatically charged for health insurance in addition to tuition and other fees. If a student already has health insurance through another source, complete the Student Health Insurance Confirmation form and turn it in to Auxiliary Enterprises on 7779 Knight Road or fax it to 713-500-8409. Students admitted to one of the regional campuses and online students should visit the Auxiliary Enterprises Insurance website at http://ae.uth.tmc.edu/info/insurance.html for more information. - Complete Library User Form
For Houston students, complete the Library User Form to activate your library account. Please read instruction carefully. Regional students must contact their respective student coordinators regarding accessibility to their library. However, all students have access to online SPH library resources. Information about these resources is available at http://www.sph.uth.tmc.edu/library.

Begin to consider pursuing the Master of Public Health degree.
Approved courses completed with an A or B while in the certificate program may be counted toward completion of the Master of Public Health degree should the student apply and be accepted. Certificate students who contemplate applying for MPH admission should review the requirements for this degree program and read the information about the divisions to which Houston applicants must apply (
MPH Degree). Certificate students considering application to the MPH program in one of the disciplines should contact the Curriculum Coordinator in that division before enrolling in the core course in that discipline.
