The Texas Council on Cardiovascular Disease and Stroke invites organizations to apply for the 2008 Texas Cardiovascular Health Promotion Awards. This program is designed to recognize outstanding cardiovascular health promotion projects in Texas.
Purpose: To recognize innovative and effective programs and policies in Texas communities for prevention and/or awareness of CVD and/or stroke programs related to high blood pressure, cholesterol, physical activity, nutrition, obesity prevention, diabetes or tobacco use will be considered for recognition.
Who may apply? Any organization involved in community health promotion efforts is eligible, including schools, worksites, health care institutions, educational institutions, public and private agencies, or any other community organizations involved in health promotion efforts.
What do winners receive? Organizations will receive recognition at the Annual Texas Public Health Association Conference and a program description on the Council’s website.
Deadlines: One original application and four (4) copies must be received by the Texas Department of State Health Services, Cardiovascular Health and Wellness Program no later than 5 PM on January 4, 2008 to the address below. Organizations will be notified of the results in February 2008.
