Applying to the Program
Admission to the Dietetic Internship is contingent upon first being admitted to the Graduate Program at the School of Public Health. Admission to the School of Public Health does not automatically assure admission to the internship, as the two processes are separate with separate applications. Incomplete applications will not be reviewed.
Tip: If you are planning to apply to the Dietetic Internship Program please first contact the Director.

Application #1: SCHOOL OF PUBLIC HEALTH
DEADLINE FOR RECEIPT OF COMPLETED SPH MATERIALS: DECEMBER 14TH OF EACH YEAR.
_____On-line SPH Admission through SOPHAS ( How to apply to SPH)
_____On-line Supplemental Application to UTHSC
_____Official Graduate Record Exam Score (SPH school codes: 6906 or 6907)
____*Notification to Dietetic Internship Director of SPH Application
*Once materials are submitted you must send notification to the DI Director of SPH Application submission and division selected by December 14th. This can be done online or through email.
Application #2: DIETETIC INTERNSHIP
DEADLINE FOR POSTMARK OF COMPLETED DI MATERIALS: FEBRUARY 14TH OF EACH YEAR

_____DI Application Form and DI $25 Application Fee, payable to UTHSCH
_____Personal Letter of Application
_____Declaration of Intent to Complete Degree Form OR Verification Statement obtained from your ADA approved Didactic Program in Dietetics**
_____Three Recommendations on ADA Forms
_____ Appointment Day Information
_____Entry into the ADA Dietetic Internship Match program submitted to D&D
